The functionality is very basic at the moment. Currently, if a reply reaches 5 recommendations it will be highlighted in the reply tree if you have the "Recommended" button selected at the top of the reply tree. The idea is that (if you want) you can leave that button turned on, and then whenever you run into a thread which has a reply with more than 5 recs, it will be brought to your attention.

However, we should be able to expand this functionality later so that highly recommended replies are featured more prominently. That will be further down the road though.
