4. Could be any of those things. Have you seen her licking those areas?

Sometimes cats will lick their fur off either as a kind of nervous habit or because of an allergy - I had a cat (Charles, RIP) who licked a patch of fur off his belly and kept licking until the skin was raw. He itched for some reason so he licked, and the licking made him itch even more and the problem got worse, so the vet gave him a cortisone shot to make the itching stop. So he stopped licking and the fur grew back. You'll probably have to take her to a vet to find out what the problem is.
