1. Poor selection criteria of officers, detective, supervisors and Chiefs
2. Poor training/education
3. Poor supervision at all levels
4. Poor/lazy investigative practices
5. Lack of accountability
6. Lack of proper Rules, Regulations, Policies, & Procedures
7. Failure to incorporate and used Best Practices
