When I booked my appointment the form said to bring an ID that would show I was in the currently eligible group (65+ or a healthcare worker at the time). I took that to mean my license or, for healthcare workers, an employer ID. Nothing was said about it having to be a 'U.S. or state' government issued ID.

(And then they never asked for my ID - I suppose a look at me was all it took to know I was eligible.)
